SUMMARY OF CLIENT’S RIGHTS

 

Persons seeking and receiving services from Corry Counseling Services, Inc. shall possess various rights including but not limited to the following:          
 

1.            You have a right to be treated with dignity and respect in all matters and by all persons associated with Corry Counselling Services, Inc.

2.            You shall not be deprived of any civil, legal, or human rights without due process.

3.            You have the right to treatment that is available or medically indicated regardless of race, color, creed, sex, ancestry, national origin, age, disability or source of payment.

4.            You have a right to be interviewed and examined in surroundings designed to assure reasonable privacy.

5.            You have the right to confidentiality regarding your treatment in accordance with guidelines established in applicable Pennsylvania laws.

6.            You have the right to expect reasonable safety insofar as Corry Counseling Services, Inc.’s practices and environment are concerned.

7.         You have the right to services that are respectful of your culture and language. 

8.         You have a right to receive a schedule of applicable fees prior to service delivery. 

9.            You have a right to know the identity and professional status of your treatment team. 

10.          You have the right to information concerning your diagnosis (to the degree known), its implications, treatment and progress.

11.          You have the right to participate in the development and review of your Treatment Plan and to reasonably participate in all decisions regarding your case.

12.          You have the right to refuse treatment to the extent permitted by law. If you or your legal representative refuses treatment, our relationship may be terminated within a reasonable period of time.

13.          You have the right to be informed of Corry Counseling Services, Inc.’s rules and regulations regarding your conduct as a client. You are entitled to information about Corry Counseling Services, Inc.’s system for initiation, review and resolution of client complaint. The Pennsylvania Department of Public Welfare regulations requires us to have a grievance and appeal system in effect to ensure that your rights are safeguarded and your concerns or complaints are promptly and fairly resolved. Copies of the grievance and appeal procedure may be reviewed or obtained at the reception desk.
