Blended Case Management

Blended Case Management services assist children and adults with significant mental health concerns to access needed supports and services. Case Managers support clients in utilizing and coordinating medical, psychiatric and dental health services; locating educational resources; applying for benefits and managing income; and securing safe and stable housing.  Blended Case Management services are client driven, strengths-based and recovery oriented. The program offers 24-hours availability as well as flexible appointment times and staff availability during non-traditional work hours.

Referral Process:

The primary treatment provider is asked to complete a brief referral form and include documentation of a diagnosis.

Eligibility Criteria: Children and adults with a diagnosis of a serious emotional illness who are in need of support in accessing treatment or other services, or in meeting essential needs.
